I'm coming in for an appointment.

Update your forms.

Spend less time in the waiting room when you fill out your forms before your appointment.

You should update this every year, or when your information changes.

Arrive 15 minutes before your appointment time.

Come early to update any necessary information. If you arrive more than 10 minutes after your appointment time, we may ask you to reschedule.

Let us know as soon as possible if you have to cancel.

Please do your best to cancel at least 24 hours before your appointment. If you cancel three times without advance notice, we may dismiss you from our practice.

I'm a new patient.

For less time in the waiting room, fill out your new patient forms before your appointment.

Get the forms.

I'm waiting on a referral.

Allow up to 7 days for your referral to go through. Once it does, our office, the insurance carrier or the specialist's office will call you with appointment information.
